PLAY DATE WAIVER
This is our liability waiver that includes COVID-19 language. All families choosing to participate in Wee Play Santa Rosa activities must agree to and sign this waiver.
I understand that it is my responsibility to supervise my child(ren) during participation in Wee Play Santa Rosa classes and play dates.
In addition, when my child(ren) participate in in-person activities, in case of an emergency when I cannot be reached, I authorize the staff of Wee Play Santa Rosa to obtain whatever medical treatment deemed necessary for the welfare of my child(ren).
I realize that personal contact with others, including but not limited to Wee Play Santa Rosa employees and participants, involves a certain degree of risk of exposure to disease-causing organisms such as COVID-19. I acknowledge that Wee Play Santa Rosa cannot screen and/or monitor all such individuals. I also recognize that all adults entering the facility, including myself, have agreed to the Wee Play Santa Rosa Illness Policy and have self-screened themselves and their children for COVID-19 at home.
I further understand and agree that I will be financially responsible for all costs incurred to treat any injuries that may occur during these activities, regardless of whether my medical insurance would cover such charges and fees.
I hereby give my consent to my child(ren)’s participation in Wee Play Santa Rosa Activities and hold Wee Play Santa Rosa harmless from any and all liability for any injuries or damages that my child(ren) or I may suffer in connection with Wee Play Santa Rosa Activities.
Social Media Notice for In-Gym Activities Only: I understand that Wee Play Santa Rosa may take photos or videos of children and/or adults in the gym. I hereby give Wee Play Santa Rosa, and its affiliates, permission for the copyright and use of these photos or videos on Wee Play Santa Rosa's social media websites.
Safety procedures/ Play Rules:
PLAY AT YOUR OWN RISK
Children must be supervised by an adult (18 yr old)/parent/guardian while in play area.
NO shoes, sneakers, high heels, flip flops, or sandals are allowed on mats (play area). SOCKS MUST BE WORN AT ALL TIMES!
NO food, drinks or sweets, or gum allowed in play area
NO running in play area
NO diving or throwing balls out of ball pit
NO equipment should be removed from within play area
NO shoving, pushing, wrestling, piling or rough play in play area.
Play areas are designed for children under 5 years ONLY.
No paint or stain/mess-prone (i.e., glitter, slime, messy arty, temporary tattoos, face paint etc.) substances are allowed in soft play areas
No more than the recommended amount of players in play area at any time
All sharp objects must be removed before play (including pens, pencils, jewelry, knives, eyeglasses, bling on pockets etc.)
DO NOT enter if you are pregnant or have physical problems.
By sending the form below, I forfeit all rights to bring a suit against Wee Play Santa Rosa and affiliates for any reason. In return, I will also make every effort to obey safety precautions as listed in writing and explained to me verbally in efforts to keep the Wee Play Santa Rosa experience positive. I have read, heard, and understand safety rules and understand that I will ask for clarification when needed.
